Roster/Waiver Forms:
All teams will need to turn in completed roster/waiver forms

before they play. You can dowload the form, Click Here.

Equipment Needed:
Stick, shinguards, mouthguards, sneakers (no cleats), & goalie equipment.  Both teams need to bring dark and light colored jerseys.

Times:
Saturday games, 9am - 7pm
Earlier start times possible if tournament is bigger.
 

Costs: 
$315 per team, includes tax & referee fees.  Receive a $25 discount for each additional tournament you register for. 

A $100 deposit is required to secure a spot.  Full payment is due prior to your first game.  Failure to pay in full may result in a $50 late fee or removal from the tournament.
